What we mean by a verified outcome

Acceptance is not a thumbs up. It is evidence, attached to a milestone, checked against criteria written up front.

Every accepted milestone carries a proof-of-work record: what was promised, what shipped, what evidence supports it and who reviewed it. That record is portable — it follows the provider's reputation and stays available to the client.

Verification is deliberately boring. Criteria are agreed at scoping, QA checks them before client review, and the client accepts against the same list. No new requirements appear at acceptance.
